What do I need to bring to my appointment?

You should bring your driver’s license, insurance card(s), any x-rays or ultrasounds, a list of current medications being taken, and your personal calendar in case of further surgery/procedure scheduling. If you have a lengthy list of current medications or just like the convenience of email send it to nurse@tomballsurgery.com.

When do I pay for my office visit/surgery?

Co-pays are expected at the time of your office visit. Co-insurance payment if known is expected prior to any surgical procedures.

What hospitals are you affiliated with?

All surgical procedures are performed at either Tomball Regional Hospital or Methodist Willowbrook Hospital.

Where can I find more information on my ailment?

WebMD is a great source for information on all ailments.

Can I get a work/school release before my appointment?

Yes, call our office during business hours. Once there is approval from the doctor it can either be faxed to your work/school or you can come pick it up.

How do I prepare short term disability paperwork?

You must first get the short term disability forms from your employer. Fill out and sign your information FIRST before bringing it to the office. By law, our office has up to 30 days to have all the information completed and returned to you.
